From the EAST (Reading)
Follow Rt. 422 West which becomes Cumberland Street in Lebanon
Turn left on 10th Street (Route 72 South)
At the intersection (red light) of Route 72 and Cornwall Road (The intersection forms a Y)
Take the left branch onto Cornwall Road and continue driving south to the intersection of Cornwall Road and Norman Drive
Turn left onto Norman Drive and look for the Lebanon Cardiology Associates sign approximately ¼ of a mile.
From the WEST (Hershey/Harrisburg)
Follow Rt. 422 East through Hershey, Palmyra, Annville and Cleona
Turn right on 12th street (Follow signs for 422 East)
Two blocks at the red light, turn left on Walnut Street (422 East)
Turn right at the red light onto 10th Street (Route 72 South)
At the intersection (red light) of Route 72 and Cornwall Road (The intersection forms a Y)
Take the left branch onto Cornwall Road and continue driving south to the intersection of Cornwall Road and Norman Drive
Turn left onto Norman Drive and look for the Lebanon Cardiology Associates sign approximately ¼ of a mile.
From the North (I-81m Exit 90)
Follow Route 72 South
Which becomes 10th Street in Lebanon
Follow 10th Street (Route 72 South)

A: Yes. |

Q: Do I need to be referred by my family doctor or another referring physician?
A: No, unless your insurance carrier requires a written referral to see a specialist. |

Q: Are you participating with my insurance carrier?
A: Check your insurance carrier website and/or manual for a listing of participating physicians. If you are still unsure, call our office and speak to someone in the billing department. |

Q: Is it okay if I bring a close friend or family member along to my visit?
A: Yes, in fact we welcome the opportunity to answer any questions or concerns your family may have about your health. |

Q: When I am having a cardiac medical problem who should I call to speak with?
A: If it is a medical emergency you would immediately dial 911 to get to the hospital, general cardiac questions/symptoms would be directed to your physicians designated LPN (Licensed Practical Nurse) to review your symptoms and relay the information to your doctor if available or to the designated triage doctor. |
