Health Insurance is designed to help you meet the cost of medical services. However, we wish to stress that the financial responsibility for services provided rests with the patient. Please inform us in advance if you are covered by a Health Maintenance Organization (HMO), Preferred Provider Organization (PPO), or Point of Service (POS). Many of these programs require advance approval to pay to medical services. We always do our best to work within the restrictions imposed by these plans; however, the patients are ultimately responsible for bringing in their referral forms on the day of their visit.

Insurance coverage's vary widely, and we cannot predict or guarantee what part of our services will or will not be covered by your particular insurance plan. Your insurance plan is an agreement between you and the insurance carrier and we look to you to pay any balances not paid by insurance. We will make every effort to help you deal with your insurance carrier. If you are eligible for Medicare, we ask that you sign an authorization form during your visit. The majority of your bill should be paid by Medicare directly to us. Please remember you are responsible for any coinsurance portion and deductibles.The fees charged are standard fees relating to the type of examination or procedure performed. Most insurance companies, except for Medicare, do not cover the fee of office visits. Therefore, payment for office visits is expected at the time of service, except for Medicare patients. There will be a minimal charge for services supervised by a physician, but not necessarily requiring his presence. For example, diagnosis and treatment without an actual office visit, such as blood pressure checks, protime checks, etc. For your convenience, we accept MASTERCARD / VISA / MAC / DISCOVER.
