Insurance

Health Insurance is designed to help patients meet the cost of medical services. However, we wish to stress that the financial responsibility for medical services provided rests with the patient. It is the patient’s responsibility to know what type of plan they have and what guidelines they need to follow to be compliant. Patients are responsible for bringing in their referral forms and/or making sure that their referring doctor obtained authorization for the medical service requested. It is also the patient’s responsibility to make sure that they notify us when their insurance changes or you get a new card.

Please inform the office in advance if you are covered by a Health Maintenance Organization (HMO), Preferred Provider Organization (PPO), or a Point of Service (POS). Many of these programs require advance approval to pay for medical services.

Insurance coverage’s vary widely, and we cannot predict or guarantee what part of our services will or will not be covered by your insurance plan. Remember, your insurance plan is an agreement between you and the insurance carrier.

 

Co-pays/Deductibles/Coinsurances

Patients are expected to pay their co-pay when they check in for their appointment. The balance of the claim will be sent out to your insurance carrier for payment.

Deductibles and coinsurance is patient responsibility for all insurance companies. As a courtesy we will submit your claim to the insurance carrier and balance bill the patient when we receive notification from the insurance carrier that it has been applied to deductible or coinsurance. Payment is due in full when your statement is received. If we have access to your deductible/coinsurance information before we provide the service, we will request payment at time of service or before a procedure is performed.

 

Past Due Bills

Payment is due upon receipt of statement. After 60 – 90 days your account with no payment activity will be considered delinquent and collection efforts will begin. Your account will eventually go to a collection agency and be reported to the credit bureau if no arrangement for payment has been made.
 

Self Pay Patients

Self pay patients are expected to pay for services rendered at time of service. You may inquire at the time of service about a discount or call the billing department before your appointment.
